What goes in a Medicaid look-back packet
A Medicaid look-back packet is the set of financial records a caseworker reviews to check for transfers made during the look-back period. It's assembled from bank, investment, and property records covering that whole window, organized so a caseworker can review it without chasing down missing pieces.
What a business office typically collects
- Bank statements for every account, for the full look-back period
- Statements for any closed accounts opened or closed during the period
- Brokerage, retirement, and investment account statements
- Life insurance policy statements, including cash value
- Real estate deeds and any transfer or sale documents
- Trust documents, if any trust exists
- Records of gifts, transfers, or large withdrawals, with an explanation for each
- Burial or funeral contract documents, if prepaid
This is a general list of document types, not legal or eligibility advice — the look-back period, required documents, and acceptable formats vary by state. Confirm the exact requirements with your state Medicaid agency before submitting a packet.
